copies of the general registration form and treatment
contract we use in our office. (You will need to have a
copy of Adobe's reader. If you do not already have it on
your computer, you can obtain it free of charge from the Adobe web-site.) We
provide these forms online as a convenience for our
patients. You can print and complete the Registration Form prior to your first appointment if you wish. We provide copies of our contracts so that you can read through them prior to making an appointment. We ask that you not sign the treatment contract until you are in our office so our staff can answer any questions and witness your signature. |
